This series consists of applications for enrollment in non-reservation schools. The applications were submitted on Standard Form 5-192a for admission to the Hayward Indian School from students attending other schools. Information in the applications may include the student's birth date; sex; degree of Indian blood; the results of a physical examination; the names of parents or guardians; and, in some cases, the previous academic record. The applications may also include a statement of consent for enrollment signed by the parent or guardian. Some files contain correspondence related to enrollment as well as receipts for travel and school supplies.

Hayward Boarding School

The Hayward Indian School in Hawyward, Wisconsin, was opened in 1901 as a boarding school for the Chippewa of the Lac Courte Oreille Reservation. It was subordinate to the La Pointe Agency until 1904. About 1911 it was given agency duties for the reservation. The school was closed in 1934, and its duties were transferred to the Lac du Flambeau Agency. ...
